Instructions for Completing the Form

1. Please complete the form to the best of your knowledge. If you have any question(s), please contact the NARO IP office (commercialization@naro.go.ug)
2. Be as specific as possible when describing the IP.
3. Include all relevant dates, such as the date of creation, the date of filing for a patent, and the date of publication.
4. If you have any questions about whether or not something should be included on the form, please err on the side of caution and include it.
5. Be sure to sign and date the form.
